In this paper, design and fabrication of a 5 kW fiber laser is reported. Manufacturing this laser is accomplished by mean of lens array combining of five 1000 W all-fiber lasers at 1080 nm wavelength. Right angle prisms are used for laser beam combination. Five output beam of lasers are coupled into a fiber with 200 µm core diameter and NA=0.22, by design and fabrication of a module and positioners with 5 degrees of freedom to hold prisms and alignment of laser beams. The final output beam quality factor M2 is measured 19.2 and 26.4 along the x- and y-axis respectively and the combining power efficiency is about 97%
